Aurora Medical Center The 1022 Club, 1022 E Sell Drive Hartford, WI 53027 (262) 673-7010 Program includes group activities and other activities chosen by the individual. There is an RN on staff. Free transportation provided within Washington County. Catholic Charities Adult Day Services 1919 N 60th Street Milwaukee, WI 53208 (414) 771-2881 Provides services to adults with disabilities. Program includes physical activities, field trips, games, and more. A CNA is available and there is a respite program. Services are provided to Milwaukee County Monday through Friday, 7:00 a.m.-5:00 p.m.

Catholic Charities Adult Day Services 13700 West National Avenue Suite 115 New Berlin, WI 53151 (262) 782-0740 Provides services to adults with disabilities and/or alzheimers and dementia. Program includes physical activities, field trips, games, and more. A CNA is available and there is also a respite program. Services are provided to Waukesha County Monday through Friday, 7:00 a.m.-5:00 p.m.

Clement Manor Adult Day Services 3939 S 92nd Street Greenfield, WI 53228 (414) 546-7366 Activities include memory games, board games, physical activities, art, and more. Services are provided to older adults in Milwaukee, Washington, Waukesha, and Ozaukee Counties.

Community Care for the Elderly 5228 W Fond Du Lac Avenue Milwaukee, WI 53216 (414) 536-2100 Program is offered to older adults enrolled in PACE or a partnership program. Activities include cognitive games, bingo, exercise, arts and crafts, and more. A full clinic is available. Participants may live in any county. Hours are Monday through Friday, 8:00 a.m.-4:30 p.m.

Activities include gardening, community outings, arts and crafts, computer skills, and more. Eight locations are available in Milwaukee, Washington, Waukesha, and Ozaukee Counties Monday through Friday, 7:30 a.m.-4:00 p.m.

Easter Seals Kindcare Adult Day Services, South Milwaukee Center 1016 Milwaukee Avenue South Milwaukee, WI 52712 (414) 571-5566 Ext. 425 Provides services to adults with disabilities Monday through Friday, 8:00 a.m.-3:00 p.m. Participants may live in any county. Program includes outings, arts and crafts, games, news, education corner, and more.

Evelyns Adult Day Care 336 Michael Court Port Washington, WI 53074 (262) 284-6768 Services provided to adults with disabilties in Ozaukee County. Program includes a variety of activities. Consumers must be able to transfer and get around unassisted.
